PHYSIOTHERAPY & THERAPEUTIC MASSAGE IN NEW MALDEN

Quick Summary

Swelling directly over the kneecap may be bursitis. Checking for infection and reducing kneeling pressure are more important than massaging the swelling. Our physiotherapists in New Malden can assess your symptoms and help you choose appropriate next steps for your goals. We agree practical goals and review your progress.

Understanding Prepatellar Bursitis

Sagittal knee anatomy showing the prepatellar bursa in front of the kneecap and the deeper fat pad.

The prepatellar bursa is a small cushioning sac between the skin and the kneecap. It can become swollen after repeated kneeling or a direct knock. Sometimes infection causes the swelling, especially if the skin has been broken. That requires medical treatment rather than routine physiotherapy.

Assessment at J&J Therapy

We ask about injury, kneeling, cuts, fever and changes in redness. Suspected infection is referred for prompt medical care. Once it is appropriate to proceed, we review knee movement and practical ways to reduce pressure during daily tasks.

Do You Experience These Symptoms?

  • A noticeable soft swelling over the front of the kneecap.
  • Tenderness when kneeling or pressing on the area.
  • Warmth or redness, which needs particular attention if worsening.

An assessment helps guide your next step, based on your symptoms, health and goals.

Why symptoms may develop

Frequent kneeling for work, gardening or sport can irritate the bursa. A skin wound, infection, gout or another inflammatory condition may also be involved. The appearance of the swelling alone is not enough to choose treatment safely.

Treatment and rehabilitation options

Hands-On Manual Therapy

Direct massage of a swollen bursa is avoided. If the cause has been checked, gentle work to surrounding muscles may help comfort, but it will not drain the swelling or treat infection. We explain the purpose and check your comfort and response throughout treatment.

Is Shockwave Suitable?

Shockwave is not a routine treatment for prepatellar bursitis, particularly when the area is acutely inflamed or infection has not been excluded. Your physiotherapist can explain the more suitable options and how their effect will be reviewed during your ongoing rehabilitation.

Targeted Exercise Programme

We help modify kneeling tasks, use appropriate padding and maintain comfortable movement. Strengthening is introduced as tolerated after the acute irritation settles. Recurrent swelling may need GP assessment of the cause rather than repeated passive treatment. Your home plan is adapted as your function and confidence improve.

When to Seek Urgent Medical Help

While most symptoms are not serious, urgent medical attention is required if you experience any of the following:

  • Seek urgent GP or NHS 111 advice for increasing redness or warmth, fever, a wound near the swelling or feeling unwell.
  • Get urgent advice if you cannot move or bear weight on the knee, or pain is severe.
  • Do not massage a hot swollen knee while infection is being considered.

Your questions answered

Can the fluid be drained?

A doctor may consider aspiration in selected cases, including testing for infection. It is not something to attempt yourself or a reason to press firmly on the swelling.

Can I continue working on my knees?

Repeated pressure may prolong symptoms. Temporary task changes, breaks and kneeling protection can help, but new redness, warmth or feeling unwell needs medical advice.

Which appointment should I book?

Start with a Physiotherapy Initial Assessment for new, persistent or recurring symptoms. We can assess the problem and agree a plan linked to your daily activities. If you have already had medical care or surgery, bring the relevant instructions.
