PHYSIOTHERAPY & THERAPEUTIC MASSAGE IN NEW MALDEN

Quick Summary

An ACL injury can affect confidence with pivoting and sport. Get a clear rehabilitation plan that fits your diagnosis, goals and any surgical advice. Our physiotherapists in New Malden can assess your symptoms and help you choose appropriate next steps for your goals. We agree practical goals and review your progress.

Understanding ACL Injury / Tear

Anterior knee diagram highlighting the ACL inside the joint and distinguishing it from the PCL and collateral ligaments.

The anterior cruciate ligament, or ACL, helps control forward movement and rotation of the shin relative to the thigh. A tear may be partial or complete and can occur with meniscus or cartilage damage. Management can involve rehabilitation alone or reconstruction followed by rehabilitation, depending on the individual.

Assessment at J&J Therapy

We review the injury history, any imaging and your orthopaedic plan. Examination and rehabilitation goals cover swelling, extension, strength and safe movement. Recurrent giving way or a locked knee may require further medical review.

Do You Experience These Symptoms?

  • A pop at the time of injury, sometimes with rapid swelling.
  • Difficulty trusting the knee during turning or changing direction.
  • Pain, reduced movement or repeated giving-way episodes.

An assessment helps guide your next step, based on your symptoms, health and goals.

Why symptoms may develop

Pivoting, a sudden stop, an awkward landing or contact injury can damage the ACL. The choice of treatment depends on instability, associated injuries, work and sporting demands, age and personal preferences rather than the scan label alone.

Treatment and rehabilitation options

Hands-On Manual Therapy

Appropriate mobilisation and soft tissue work may support comfortable movement alongside exercise. After reconstruction, all techniques respect the graft, healing tissues and surgical protocol. Hands-on care cannot substitute for rebuilding strength and control. We explain the purpose and check your comfort and response throughout treatment.

Is Shockwave Suitable?

Shockwave is not a routine treatment for an ACL tear or graft rehabilitation. It does not replace the decision about stability, surgery or a structured exercise programme. Your physiotherapist can explain the more suitable options and how their effect will be reviewed during your ongoing rehabilitation.

Targeted Exercise Programme

Rehabilitation develops knee movement, strength, balance and movement confidence. Later stages test running, landing and turning capacity where relevant. Returning to pivoting sport requires a shared, criteria-based decision and attention to psychological readiness, not simply waiting a set number of weeks.

When to Seek Urgent Medical Help

While most symptoms are not serious, urgent medical attention is required if you experience any of the following:

  • Seek urgent GP or NHS 111 advice if you cannot move the knee, bear weight, or straighten a knee that is locked.
  • A hot swollen knee with fever or feeling unwell needs urgent medical assessment.
  • Go to A&E for a deformed leg after injury or a foot that becomes cold, pale or numb.

Your questions answered

Does every ACL tear need surgery?

No. Some people manage well with structured rehabilitation. Ongoing instability, associated injuries and a wish to return to demanding pivoting activities can change the discussion.

Can I have physiotherapy before surgery?

Yes, when the medical team agrees. Preparing knee movement, strength and a practical post-operative plan can be valuable, while respecting any associated injury restrictions.

Which appointment should I book?

Start with a Physiotherapy Initial Assessment for new, persistent or recurring symptoms. We can assess the problem and agree a plan linked to your daily activities. If you have already had medical care or surgery, bring the relevant instructions.
