What Is Shoulder Impingement?
Shoulder impingement is pain arising from the soft tissues in the subacromial space — the rotator cuff tendons and the subacromial bursa that sit beneath the bony arch (acromion) at the top of your shoulder. The traditional model held that these tissues were being mechanically squeezed when you lift your arm.
Modern evidence has shifted this understanding. Because pain does not correlate well with the size of the subacromial space, clinicians increasingly favour the terms subacromial pain syndrome or rotator cuff related shoulder pain. Current understanding is that the pain is often related to tendinopathy — a load-related change in the rotator cuff tendons — rather than purely mechanical pinching (Lewis, British Journal of Sports Medicine, 2009).
Rotator cuff disorders account for around 85% of shoulder pain presenting in primary care, and the condition typically peaks in the fifth and sixth decades of life. Repetitive overhead work or sport raises the risk. Research suggests shockwave therapy can reduce pain and improve shoulder function in rotator cuff tendinopathy (Xue et al., BMC Musculoskeletal Disorders, 2024).

✓ Pain on the outer or front of the shoulder, often when reaching overhead or out to the side
✓ A "painful arc" — discomfort through the mid-range of raising your arm that eases at the top

What Causes Shoulder Impingement?
• Tendon overload and tendinopathy
The most common driver is load-related change in the rotator cuff tendons, especially the supraspinatus.
When demand on the tendon exceeds what it is conditioned for, it can become painful and sensitised.
• Repetitive overhead activity
Jobs and sports involving sustained overhead reaching — painting, construction, swimming, racquet sports — increase mechanical load on the shoulder.
This helps explain why symptoms often build gradually over time.
• Age-related tendon change
Rotator cuff tendons naturally degenerate with age, and the condition peaks in the fifth and sixth decades of life.
These changes reduce the tendon’s tolerance to load, though they are common even in people without pain.
• Weakness and altered movement control
Reduced strength in the rotator cuff and the muscles controlling the shoulder blade can change how the shoulder moves under load.
Addressing this control and capacity is a central aim of rehabilitation.
• Lifestyle and health factors
Smoking, poor sleep and higher body weight have been associated with a greater likelihood of rotator cuff symptoms.

Targeted Exercise Programme
Progressive, individually tailored exercise is the cornerstone of evidence-based care for this condition. Our physiotherapists design a programme that gradually loads and strengthens the rotator cuff and shoulder-blade muscles, restoring capacity and confidence in the movements that provoke your pain. Evidence shows a well-guided programme you can carry out largely at home is as effective as more intensive treatment.
When to Seek Urgent Medical Help
While most symptoms are not serious, urgent medical attention is required if you experience any of the following:
Significant trauma with pain and weakness, or a sudden inability to actively lift your arm — this may indicate an acute rotator cuff tear
A hot, red, swollen and painful shoulder, especially with fever or feeling generally unwell — this may indicate infection
An obviously deformed shoulder after injury, with loss of movement — this may indicate a dislocation needing emergency care
A new, unexplained lump or swelling, unexplained weight loss, night sweats or a history of cancer — these warrant urgent medical review
FAQs About Shoulder Impingement
Do I need surgery for shoulder impingement?
Usually not. High-quality placebo-controlled trials show that keyhole "decompression" surgery is no better than placebo surgery, and no better than exercise therapy, for this condition (Beard et al., The Lancet, 2018; Paavola et al., BMJ, 2018). This is why guidelines recommend trying structured physiotherapy first.
Can shockwave therapy help my shoulder?
Research suggests it can. For rotator cuff tendinopathy, shockwave therapy has been shown to reduce pain and improve shoulder function, and it may be particularly useful where there is calcific tendinopathy (Xue et al., BMC Musculoskeletal Disorders, 2024). We use the Swiss Storz Medical MASTERPULS radial device, and a typical course is 3–6 sessions delivered alongside your exercise programme. It is non-invasive and generally well tolerated.
How long will it take to get better?
Most people improve with consistent rehabilitation, but shoulder pain can be slow to settle. Evidence suggests it can take at least 12 weeks of regular exercise to feel meaningful gains, and some people take several months. Being consistent with your programme gives your shoulder the best chance of recovery.
What can I do to help myself?
Keep the arm gently moving within comfortable limits, avoid complete rest, and gradually return to activity rather than pushing into sharp pain. Your physiotherapist will guide how much load is right and when to progress.
Is imaging (a scan or X-ray) needed?
Often not initially. Scan findings such as tendon changes are common even in pain-free shoulders, so they do not always explain symptoms. We diagnose clinically and reserve imaging for cases that do not respond as expected.
